Documents for the IRB application
In accordance with campus regulations, all participating faculty will need to acquire approval form the IRB. The deadlines for submitting applications will vary across campuses and each faculty member should be aware of these dates. Though the forms and procedures will vary from campus to campus, we have provided resources that can be edited and adapted to fit the specific requirements of each campus. For the purpose of consistency across all tests, we have designed the surveys and data requests. If you want or need to make changes to these forms please contact the Ithaka S+R team. We will try to accommodate any requested changes but we will also need to ensure some level of standardization across test cases.
